This form must be completed and returned to the appropriate Division Convener for any requested tournaments during League play no later than October 31 2025. Please be advised that this form is not necessary for Tournaments within CRRL. Gary Field and COLT do not require Blocking. League Play occurs between October 17, 2025 and March 2026
